SOAP Notes (Scribe) for veterinary practices

The 6:30 exam wraps up, and
today's records are already drafted.

SOAP Notes is an AI scribe that drafts veterinary SOAP notes from the exam-room conversation, for DVMs at US vet practices. You talk with the pet parent as usual. The draft is ready before your next appointment, and you edit and sign it.

  • A draft is ready before the next exam
  • Your DVM signs each one
The day's SOAP notes in CompanionBooked, each drafted minutes after its exam and marked draft, to sign or signed.

6:40 p.m. The last pet parent has gone, the techs are mopping exam room two, and the DVM has six records open with eleven callbacks after them. Most of that evening goes on typing what was said hours ago. Below, the same evening once the scribe drafts each note in the room.

How the evening goes now

Before CompanionBooked: a pile of six handwritten exam records still to type and sign at 6:40 p.m., with a sticky note of eleven callbacks.

Three steps

How the scribe gets into your exam rooms

Your DVMs try it on real appointments before launch.

Dr. Holt's note format in CompanionBooked: four signed records she shared, and the shorthand, vitals line, dental charting and plan order the scribe copies from them.

Your DVMs send a few signed records they like, and we copy that format: the abbreviations you use, how dentals are written, what goes in the plan. Each DVM can keep their own.

In your week

Four records that take less of the night

  • 6:40 p.m.

    The evening pile

The last exam ends at 6:00, and the DVM still owes six records and a list of callbacks. Most nights that's an hour or two of typing from memory. With the scribe, each note was drafted in the room, so the evening is reading and signing.

    • Drafts are waiting the minute the exam ends.
    • Signing takes a read-through, not a rewrite.
The SOAP notes list in CompanionBooked at the end of the day: six notes from three doctors, each drafted in the exam room and waiting only for a signature.
  • Relief DVMs

    One layout across your DVMs

A relief vet writes one way, the owner another, and the recheck goes to whoever is free. The scribe drafts every DVM's notes in your practice's format, so anyone can pick up the case.

    • Your abbreviations and headings, kept as is.
    • Each DVM can keep a template of their own.

  • Intake forms

    The history, filled in early

A new pet parent books online and fills in the intake form by text the night before: diet, past vaccines, the limp that started Friday. The scribe adds it to what's said in the room, so the history is complete when you sign.

    • Intake answers carried into the history for you.
    • Forms go out the moment a pet parent books online.
Ziggy's record in CompanionBooked before his first exam: the intake form Marcus filled in by text the night before, his shelter vaccines, and the history already drafted into the SOAP note.
  • Checkout

Mid-exam extras, written into the plan

Halfway through an exam you add a nail trim and a fecal test, and by checkout nobody remembers. The scribe writes every test, med and procedure you mention into the plan, so your CSR rings up the appointment from a full note.

    • Vaccines, tests and meds listed in the order you gave them.
    • The recheck date in the plan tells the desk what to book next.

Scribe FAQs

Asked by DVMs before a demo

Consent, templates and PIMS are the words DVMs type most.

6 questions

Who drafts the note?

An AI scribe on your phone records the exam, turns the talk into text and drafts the SOAP note, ready for you to edit and sign.

What if the draft has a mistake in it?

You read every draft before signing, so nothing reaches the record that you haven't checked. Fix any line with a tap, then sign.

Do I need consent to record exams?

Some states require everyone in the room to agree to a recording, so your new-client form asks for it.

Does the note go into our PIMS?

Yes. Once you sign it, the note is filed in the pet's PIMS record.

Can I edit the note before it's saved?

Yes, every note stays a draft until the DVM signs it. Change, add or delete anything first, and only the signed version is saved.

What's in a good vet SOAP note?

A good veterinary SOAP note covers four parts: what the pet parent reports, your exam findings, your assessment and the plan.
